Application for Homebound Services

Homebound Services is a free monthly delivery service of Ottawa Public Library materials that is offered to Ottawa residents. If you are unable to regularly visit a library branch due to age, illness, or disability you may be eligible to register for this service.

The personal information requested for your library card is collected under the authority of the Public Libraries Act. The information collected will be used in the day-to-day operations of the library.

Personal information provided is collected under the authority of Sections 20(a) and (d)  and 23(2) of the Public Libraries Act. Personal information is used to provide library services. The Ottawa Public Library (OPL) is subject to the privacy requirements under the Municipal Freedom of Information and Protection of Privacy Act.
